Services provided
One Form Wellness provides Online Naturopathy consultations, face to face personal training sessions and online training supporting your health holistically on the inside and outside.
Client Obligations
As a client of One Form Wellness, you have responsibilities such as attending your consultation/sessions regularly and on time, following your practitioner and/or trainer’s instructions, and providing accurate health and medical information. It is highly important that you inform your practitioner and/or trainer of any health concerns or changes in medical conditions, including ongoing treatments or medications.
Booking and payment
All bookings are made through the clinic management software, Simple Clinic. Full payment is required prior to your naturopathy consultation or personal training session.
For personal training, invoices are charged two weeks prior to your sessions on a recurring invoice, and you have up to 7 days to pay. If you fail to pay within 7 days, you will receive a warning. Furthermore, if it happens again, there will be additional charges and suspension of services. If you are going away, please notify hello@oneformwellness.com at least 2 weeks before so your invoice can be adjusted or payment paused if needed. If you fail to notify before the next invoice, you may be charged for your usual sessions, and you will have session credits that you can use when you return.
An additional processing fee of 2% of the total amount per invoice will be added if you wish to pay by invoice in order to cover merchant fees.
Late arrival
Clients arriving late to either naturopathy consultation or personal training session will receive the remaining scheduled session time, unless other arrangements have been previously made with the practitioner or trainer.
Naturopathy Cancellation Policy
One Form Wellness has a 48 hour cancellation/reschedule policy for all naturopathy consultations: If you cancel/reschedule your appointment within 48 hours, you will forfeit 100% of your prepayment for the consultation.
We appreciate your understanding as this allows us to schedule in other clients to fill your spot.
Personal training Commitment Period and Termination Policy
We require a 4-week minimum commitment period from your first session and at least 2 weeks’ notice period to terminate your weekly sessions.
In the case of termination, there will be no refunds for unused or prepaid sessions.
Personal training Cancellation policy
One Form Wellness has a 24 hour cancellation/reschedule policy for all naturopathy consultations: If you cancel/reschedule your appointment within 24 hours, you will forfeit 100% of your prepayment for the session.
For example, if you have a session at 4pm on a Wednesday, let me know before 4pm on Tuesday if you can’t attend.
If you let me know that you are sick prior to the 24 hours, sessions will remain as a credit on your account, and you can organise a time to do a ‘make-up’ session with your trainer. If credits build up to more than 6 sessions on your account, payments will be paused and you will be forced to use all your credits.
Please be considerate and let me know as soon as you are feeling sick so that I can schedule in another client who may want your spot.
If you go away, you have the option of pausing your training or you can switch to online training and continue your training program and support with nutrition. If you wish to pause, your program will put on hold and extended for the length that you were away and you can finish it when you return.
Use of Images, Videos and Testimonials
As part of the process, you may be required to take photos of your body as this is a great way to track your progress. For online clients, you will be asked to take videos of yourself performing various exercises at the gym so that I can correct your form via video analysis.
By signing this form, you are agreeing to the use of these “before and after” transformation photos, exercise videos and testimonials for promotional and marketing materials of One Form Wellness.
Health and Liability
Please consult with a medical professional before commencing the naturopathy treatment plan to ensure it is safe and appropriate for your specific health conditions. For any injuries or adverse reactions that may occur during or as a result of the treatment, please refer to the liability waiver.
Please consult with a medical professional before commencing the training program to ensure you are fit for physical exercise. For any injuries or accidents that may occur during training sessions, please refer to the liability waiver.
It is recommended that all clients obtain appropriate insurance coverage for personal injury or loss.
Confidentiality
All client information, including personal and health-related data, will be kept confidential and not shared without prior consent. There is exceptions to confidentiality, such as when required by law or for professional consultation purposes. Please refer to the Privacy Policy for more information.
Intellectual Property
All naturopathic materials, treatment plans, and intellectual property provided by the naturopath remain their exclusive property. All training materials, programs, and intellectual property provided by the personal trainer remain their exclusive property.
Governing Law and Jurisdiction:
Jurisdiction and governing law will apply to any disputes arising from the agreement.
Shipping Policy
All purchases and/or services processed via One Form Wellness, Squarespace and authorised platforms are stated in Australian Dollar (AUD$). All orders are dispatched from Sydney, Australia.
Your items will be delivered via Australia Post and/or Vitaly who intend to deliver your items efficiently. Once your order has been dispatched, you will receive a confirmation email including tracking information.
One Form Wellness does not take responsibility for any damaged, lost, missing or delayed items.
Refund Policy
Please note we do not offer refunds on any naturopathy or personal training service provided or naturopathy product purchased. This includes but not limited to relocation, illness and unused sessions.
Amendments
The terms and conditions may be modified from time to time and clients will be notified of any changes.
Severability
If any part of this agreement is found to be invalid or unenforceable, the remaining terms will still be valid.
